In the previous episode, we saw that Rena has been continuously losing her sanity since she was injected with H-173 by Satoko. Later, after a lot of problems, Keiichi says that he is just being Paranoid. Later, Keiichi hugs Rena as she comes over to cook for him.
When Rika thinks that everything is fine, we saw that Rena was continuously stabbing Keiichi. Keiichi trying to defend himself hits her repeatedly with a box. The next day, Rika gets to know the whole story. But due to the previous fight, Rena had died and Keiichi was badly injured and was admitted to the hospital.
